Provides consultancy to: Commonwealth Home Support Program (cHSP) and HACC Program for Younger People (HACC PYP) funded district nursing services and public sector residential aged care services (PSRACS - high level care residents) Avoca, Bendigo, Boort, Castlemaine, Cohuna, Dingee, Dunolly, Echuca, Gisborne, Heathcote, Inglewood, Kerang, Kyabram, Kyneton, Lockington, Maldon, Manangatang, Maryborough, Mildura, Murrayville, Nyah West, Ouyen, Patchewollock, Pyramid Hill, Quambatook, Red Cliffs, Robinvale, Rochester, Rushworth, Sea Lake, Swan Hill, Underbool and Woodend.

Referrals for Consultation Service: Complete referral form (available to download from link below) and fax to the Bendigo Health Referral Centre 54547099.
